The Enrolment Process
Complete the enrolment form below and you will be contacted by our team when your enrolment has been considered. We are operating on a one in one out basis

Why enrol with us
Enrolment is easy, costs nothing and benefits you, your family and your community.
When you enrol, you pay less for visits to your regular doctor.
In addition to this, your doctor’s practice will help you take part in prevention programmes, such as cervical and breast screening, and it will send you immunisation reminders for your children.
The practice will help you to manage conditions such as diabetes and high blood pressure and to address other health-related issues, such as smoking, to achieve a healthy lifestyle.
Benefits of being enrolled
There are many benefits to enrolling us. These include:
Children under the age of 14 get free healthcare
You pay less when you are an enrolled patient
You get subsidised medicine when it is prescribed for you
Access to free health programmes such as quit smoking or fitness
Free medical advice after hours, on weekends and public holidays
Community services card holders can get reduced fees when visiting a general practice
Eligibility
To be eligible to enrol as a patient, you must be a NZ Citizen or a New Zealand Resident.
If you are a New Zealand citizen (this includes people from the Cook Islands, Niue and Tokelau) or resident, you are eligible for the full range of publicly funded health and disability services.
This is also generally the case if you hold a residence class visa; If you hold a work visa that allows you to stay in New Zealand for at least two consecutive years; or you are an Australian citizen or permanent resident staying in New Zealand for two years or more.
International students (on a student visa) are not eligible to enrol and should make sure they have medical insurance.
How to enrol
When you enrol you will need to provide identification that confirms your identity and residency status.
If you are changing general practices, your medical records will be transferred to us upon completion of enrolment forms.
Please note you can only enrol with one general practice.
What is a PHO?
A Primary Health Organisation (PHO) is a not-for-profit group funded by Health New Zealand | Te Whatu Ora to ensure that people in a specific region have access to essential primary health care services. PHOs work with general practices and other health providers to deliver subsidised care, promote health, and improve access to services—especially for communities with greater health needs.
Most New Zealanders are enrolled with a general practice that is part of a PHO. This enrolment helps reduce the cost of doctor visits and provides access to a wider range of health services.

What is General Practice?
General practice provides comprehensive, community-based, patient-centered healthcare. Services include:
Diagnosis and management of health conditions
Ongoing care throughout life
Health promotion and prevention
Screening
Referrals to hospitals and specialists
Enrolling with a General Practice
What happens if I go to another practice?
You can visit or change to another general practice at any time. However, if you are enrolled with a PHO through one practice and visit another as a casual patient, you will likely pay a higher fee for that visit. You can only be enrolled with one general practice at a time.
What happens if the general practice changes to a new PHO?
If your general practice changes its PHO affiliation, they are required to inform you of this change.
What happens if I am enrolled but don’t visit often?
If you haven’t used services from your general practice in three years, they may contact you to confirm whether you wish to remain enrolled. If they can’t reach you or you don’t respond, your name may be removed from the practice and PHO enrolment registers. You can re-enrol at any time with the same or a different general practice.
Benefits of Enrolling
Enrolment is free and voluntary. If you choose not to enrol, you can still access health services, but:
